The review identified a series of significant governance and operational issues, including non-compliance with funding agreements, such as retaining unexpended grant money, systemic control failures, underpayment of staff against applicable awards, and inadequate reporting to the Board. These weaknesses increased financial, regulatory, and operational risks for the organisation.
The Financial Controller was transitioned out of the organisation in a managed and controlled manner, ensuring continuity of essential health services throughout the process and supporting the recruitment of a suitable replacement.
